I just sent another version which has your preferred page headings.

However, I just realised that you can have this much easier.  The 
page-filling does not depend on the document class, but only on whether 
you chose the "twoside" or "oneside" option.  And because I stupidly 
chose "twoside" to match what "book" does by default, my just-sent 
patch is worthless.  Please ignore.
