One more question, thanks to Jamie:

11. Did you use the --external-merge option? Keep in mind that this may
    have been in your ~/.darcs/defaults.

I've found a bug in the external merge code that shows up in certain cases
when a subdirectory was created by a patch.  I'm not sure if this what
fraction of the cases this bug accounts for, which is why I'm asking this
question.
