I have narrowed down where this bug happens.  It is in the
optimization of apply_list in PatchApply.lhs.

I have not wrapped my brain around the code, so I don't
understand why it happens.  I only supply a patch that reverts
the optimization as a proof of concept.  Unfortunately it
also reverts the set-script-executable functionality, so it
should not be applied.
