Do you think that the errors reported by mh2maildir might also be the
same problems that Evolution is having issues with?

I'm not sure I understand what you mean by having "files directly in
each folder" - Maildir has a single message per file format. Also I
don't think there is a specific requirement to have those subfolders -
it might be just how Evolution displays it's virtual folders, although
I think Inbox is a standard name for mail in the top level Maildir
directory. (I have a 'Junk' folder in my local Maildir store on
Evolution, but there is no corresponding directory on the disk.)

Disable IPv6 on your machine to see if it makes a difference to the
IMAP speed.

The timeout that others have seen is because the system uses IPv6 by
default and it takes a while for it realise that the packets aren't
getting through, it then falls back to IPv4 and it works. It may be
something specific to the route to the IMAP server.
