in /(ht)docs/user.php in the action "create_mysql_user", validation begins:

    //If we are using ldap_auth, make sure username isn't already in LDAP
    if ($conf['ui']->ldap_auth()) {
            if (!$user->check_auth_name($username)) {

                    $conf['error']->add_error('username', "Username already exists in LDAP.");
                    $action = 'show_add_user';
    
            } //check auth name
    } // if ldap_auth()

Problem:

$user->check_auth_name($username) returns true if the name exists.  therefore:

!$user->check_auth_name($username) will be true if the name does not exist, counter to the error message and program flow.

Fix simply requires removing the "!"
